Factores que conducen a la obstinación terapéutica en adultos mayores en cuidados paliativos: revisión sistemática

Introducción: La obstinación terapéutica, (también llamada “encarnizamiento terapéutico”), describe la persistencia de intervenciones desproporcionadas sin beneficio clínico relevante en pacientes en fase avanzada o terminal. En adultos mayores con enfermedades crónicas no oncológicas, su presencia genera conflictos clínicos, éticos e institucionales que afectan negativamente la calidad del proceso de morir. Objetivo: Analizar los factores clínicos, institucionales, éticos, culturales y profesionales que conducen a la obstinación terapéutica en adultos mayores que reciben cuidados paliativos no oncológicos. Metodología: Revisión sistemática cualitativa conforme a PRISMA 2020. Se incluyeron 30 artículos publicados entre 2009 y 2025, seleccionados mediante criterios PICo. Se analizaron estudios observacionales, cualitativos y revisiones. La calidad metodológica de las revisiones fue evaluada mediante AMSTAR 2, NOS, JBI, CASP y coherencia interna y pertinencia. Protocolo definido a priori; no registrado en PROSPERO. Resultados: Los factores que favorecen la obstinación incluyen: ausencia de planificación anticipada, el modelo biomédico dominante, escasa formación paliativa y bioética, desconocimiento de la autonomía y presiones socioculturales - religiosas. Conclusiones: La obstinación terapéutica es multicausal y requiere estrategias integradas: abordaje temprano de cuidados paliativos, decisiones compartidas y fortalecimiento formativo y organizacional.
